    Grondoorzaak: Lata zaniedban administracyjnych: brak relokacji niebezpiecznego materialu mimo wielokrotnych ostrzezen. Spawanie w sasiedztwie niewlasciwie wentylowanego skladu NH4NO3. Wieloletnia korozja workow umozliwiajaca caking i wzrost reaktywnosci.

    Geleerde lessen: Niepalne konstrukcje + automatyczne sprinklery dla magazynow >5 ton NH4NO3. Wymog separacji od materialow palnych >15 m. ATF/OSHA jurisdiction gaps musza byc zamkniete legislacyjnie.

    Geleerde lessen: Double-block-and-bleed valve standard dla high-pressure reactor isolation. Kierunek pneumatic actuators musi byc fail-safe. OSHA PSM standard 29 CFR 1910.119 (1992) jest bezposrednim wynikiem tego incydentu.
